Mental Health Reference Nurse - APREMADOL
Our Medical-Social Centers (CMS) offer multidisciplinary services of quality in the West Lausanne area, and their interventions are based on values of humanity and equity. To complete our teams, APREMADOL is looking for immediately or by agreement: Mental Health Nurse Referent (70%-80%) Your mission As a referent nurse, you will be responsible for ensuring quality services to guarantee the health, safety, and well-being of clients at home. In their living environment, you define the most suitable solutions with them, thus developing a privileged trust relationship. Discover more about this profession by watching the video linked to this description.Responsibilities - Develop the care project based on the needs of the client and their surroundings; provide the services planned in the intervention plan. - Actively participate, in an interdisciplinary manner, in the management and follow-up of cases in collaboration with care and housekeeping staff. - Ensure delegated medical care, hygiene, and comfort, as well as support for our clients. - Receive, clarify, and direct requests. - Document the client's file according to institutional directives to ensure continuity of care. - Support aides to guarantee the quality of services and maintain lasting relationships with regional medical-social network partners (including volunteers and user groups). - Act as a referent in following up on clients and in providing advice, support in administrative and financial processes. Your profile and skills: - Bachelor's degree in nursing or equivalent title (essential), with specialization in mental health. - Minimum of 6 months of experience in mental health (essential). - Good knowledge of the Swiss socio-sanitary field. - Ability to analyze and synthesize. - Good command of the French language with strong writing skills. - Ability to manage priorities and knowledge of standard tools. - Ability to communicate and negotiate. - Ability to work in a multidisciplinary team. We offer: - Mainly daytime hours (7:30 AM - 5:00 PM), with only one evening shift worked every two weeks until 10:00 PM and one weekend worked every eight weeks. - Working conditions and benefits in line with the public health sector's collective bargaining agreement, including quality professional coverage, beyond minimum legal requirements. - 4 weeks of vacation per year, increasing to 6 weeks after 50 employees. - Opportunities for continued education and advancement within the network. - A stimulating and humanely rich professional environment, serving the well-being of clients.
